What is Spravato and how much does Medicare pay for it?

Spravato (esketamine nasal spray) is an FDA-approved treatment for treatment-resistant depression. Under Medicare Part B, clinics buy the drug and bill per session — roughly $900 to $1,300 of Medicare payment per session depending on the dose.

What does sessions per patient mean?

Total Spravato sessions a provider billed in a calendar year, divided by the number of Medicare patients they treated. A provider following the FDA label — twice weekly for a month, weekly for a month, then every one to two weeks — lands near 34 sessions per patient. A provider that never tapers anyone caps out around 56.

Why are some providers flagged?

Anything above 56 sessions per patient is impossible under the label without keeping patients on twice-weekly dosing all year, which is the profile federal auditors look for. We also flag providers whose billing type is not a psychiatric specialty, providers billing more sessions than patient-days (same-day duplicates), and group NPIs where the ratio blends several prescribers.

Does this cover all Spravato billing in the US?

No. It covers Medicare fee-for-service only — Medicare Advantage (more than half of beneficiaries) and pharmacy-benefit dispensing are not in the file. CMS also suppresses any provider-code combination with fewer than 11 patients.
